Pharmacology
Doxycycline Hydrochloride:
A broad-spectrum tetracycline-class antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit. It is bacteriostatic and active against a wide range of Gram-positive and Gram-negative organisms, as well as atypical bacteria.
Lactic Acid Bacillus:
A probiotic containing beneficial bacteria (usually Lactobacillus sporogenes or Bacillus coagulans) that restores the natural intestinal flora disturbed by antibiotic use. Helps in preventing or treating antibiotic-associated diarrhoea and gastrointestinal disturbances.
Indications
Docylac Capsule is indicated for:
- Pelvic Inflammatory Disease (PID)
- Bacterial infections including respiratory, urinary tract, skin, and sexually transmitted infections
- Gastrointestinal protection during antibiotic therapy
- Traveler’s diarrhoea
- Acne, periodontal infections, and pelvic inflammatory disease
- Typhoid fever and cholera
- Rickettsial infections, malaria prophylaxis (off-label use)
Dosage
Adults:
- 100 mg (1 capsule) once or twice daily depending on infection severity
- Taken with plenty of water to avoid esophageal irritation
- Avoid lying down for at least 30 minutes after taking
Note: Dosage duration depends on indication (e.g., 7–14 days for bacterial infections)
Precautions
General
- Take with a full glass of water
- Do not take with milk or antacids containing aluminium, calcium, or magnesium
- Probiotic helps prevent gut flora imbalance
Warnings
- May cause photosensitivity – avoid excessive sunlight or tanning beds
- Can lead to esophagitis or ulceration if taken without water or lying down immediately after
- Use with caution in hepatic dysfunction
Contraindications
- Hypersensitivity to doxycycline, tetracyclines, or any component of the formulation
- Children <8 years (due to permanent tooth discoloration and bone growth suppression)
- Pregnancy and lactation (relative contraindication)
Drug Interaction
- Reduced absorption with antacids, calcium, magnesium, iron, and zinc
- May enhance anticoagulant effects of warfarin
- May reduce the efficacy of oral contraceptives
- May increase plasma levels of digoxin
- Use of lactic acid bacillus with antifungals or other antibiotics may reduce probiotic efficacy
Pregnancy
- Category D: Evidence of foetal risk, but may be acceptable in life-threatening infections. Avoid unless clearly indicated.
- Lactic Acid Bacillus is generally safe but not enough data during pregnancy when combined.
Adverse Effects
- Generally, well tolerated.
- Possible rare effects Mild GI discomfort (bloating or diarrhoea initially due to probiotics)
Allergic reaction to herbal or fruit extracts
Soft stools (from magnesium or prebiotic action)
Overdosage
- Doxycycline Overdose: Symptoms: Nausea, vomiting, dizziness, headache Management: Symptomatic and supportive care; no specific antidote
- Lactic Acid Bacillus: Extremely safe; overdose unlikely to cause harm
Storage
- Store in a cool, dry place, below 25°C
- Protect from moisture and light
- Keep out of reach of children
