Pharmacology
Ferrous Bisglycinate is a highly bioavailable and well-tolerated chelated iron-amino acid complex in which two glycine molecules are bound to a single iron atom. This unique structure allows it to be recognized as an organic compound by the body, enhancing its absorption and utilization. Unlike traditional iron salts, Ferrous Bisglycinate is absorbed intact by enterocytes in the duodenum via amino acid transporters. Within the enterocytes, it is hydrolyzed to release iron, minimizing exposure to the gastrointestinal tract. As a result, common side effects associated with conventional iron supplements—such as gastrointestinal irritation, constipation, food interactions, and pH-dependent absorption—are significantly reduced. Additionally, it helps lower oxidative stress and gastric discomfort, making it a gentler and more effective option for iron supplementation. 

Zinc Bisglycinate:​ A chelated form of zinc with superior absorption and GI tolerance. Essential for enzymatic functions, immune support, wound healing, and cellular metabolism. 

Folic Acid (Vitamin B9):​ Involved in DNA synthesis, red blood cell formation, and neural tube development during pregnancy. 

Methylcobalamin (Vitamin B12 active form):​ Essential for neurological function, DNA synthesis, and red blood cell maturation. It’s the neurologically active form of Vitamin B12, used in peripheral neuropathy, anaemia, and fatigue.

Indications

FeB9 is indicated for: 

  • Iron-deficiency anemia 
  • Megaloblastic anemia 
  • Anemia of pregnancy and lactation 
  • Post-surgical anemia 
  • Nutritional deficiencies in folate, B12, or zinc 
  • Fatigue, weakness, and general debility 
  • Neuropathy support (especially in diabetic or nutritional neuropathy)
Dosage
  • In Treatment of anemia: 1 tablet twice a day (BID)
  • In Prevention of anemia: 1 tablet a day (OD), or as directed by a healthcare professional.
  • Can be taken before or after meals.
Precautions
General 
  • Ensure no co-existing iron overload disorders 
  • Prolonged use should be monitored with serum iron, folate, B12, and zinc levels 
  • Avoid taking with calcium or dairy (may reduce iron absorption) 
Warnings 
  • Excess iron may lead to iron overload in susceptible individuals (e.g., hemochromatosis) 
  • High-dose folic acid can mask B12 deficiency, so combination is appropriate 
  • Zinc at high doses over time may lead to copper deficiency 
Contraindications
  • Known hypersensitivity to any component 
  • Conditions with iron overload (e.g., hemochromatosis, hemosiderosis) 
  • Severe renal impairment (monitor zinc and B12 accumulation) 
Drug Interaction
  • Iron absorption may be decreased by: Calcium, tetracyclines, quinolones 
  • Zinc may interfere with absorption of: Tetracyclines, fluoroquinolones (space by 2 hours) 
  • Vitamin B12 absorption can be reduced by: Proton pump inhibitors and metformin (long-term use) 
  • Folic acid efficacy may be reduced by: Methotrexate, trimethoprim, sulfasalazine
Pregnancy
  • Safe and recommended during pregnancy and lactation 
  • Supports foetal growth, neural tube development, and prevention of anemia
Adverse Effects
  • Ferrous Bisglycinate: 

- Mild gastrointestinal symptoms (rare compared to ferrous sulphate) 

- Constipation or dark stools 

  • Zinc Bisglycinate: Occasional nausea or metallic taste 
  • Folic Acid: Rare: nausea, allergic reactions 
  • Methylcobalamin: Well tolerated; rare rash or dizziness
Overdosage

Iron Overdose:
∙Nausea, vomiting, abdominal pain, diarrhoea 
∙Severe cases: metabolic acidosis, shock, liver damage 
∙Treat with iron chelators (e.g., deferoxamine) if necessary 
Zinc Overdose: 
∙Nausea, vomiting, abdominal cramps 
Folic Acid & B12: 
∙Low toxicity; overdose is generally not harmful

Pharmacology
L-Methylfolate: 
The active form of folate that crosses the blood-brain barrier. It plays a role in the remethylation of homocysteine to methionine and is essential for DNA synthesis, repair, and methylation. 

Pyridoxal 5-Phosphate (P5P): 
The bioactive coenzyme form of Vitamin B6. It functions in amino acid metabolism, neurotransmitter synthesis, and modulating homocysteine levels. 

Methylcobalamin: 
The neurologically active form of Vitamin B12. It contributes to nerve cell health, red blood cell production, and DNA synthesis. It also aids in the conversion of homocysteine to methionine.

Indications
  • Management of hyperhomocysteinemia 
  • Neuropathies (diabetic neuropathy, peripheral neuropathy) 
  • Megaloblastic anemia due to Vitamin B12 or folate deficiency 
  • Support in mood disorders like depression (especially when folate deficiency is involved) 
  • Adjunct in cardiovascular health 
  • Nutritional supplementation during pregnancy, lactation, or chronic illness
Dosage
  • Typically, one tablet daily, or as directed by a physician. 
  • Can be taken with or without food.
Precautions
General 
  • Use with caution in individuals with a history of folate or B12 metabolism disorders. 
  • Monitor homocysteine levels if used long-term for cardiovascular or neurological conditions. 
Warnings
  • High doses of B vitamins, especially B6, may cause peripheral neuropathy over prolonged use. 
  • Monitor patients with renal impairment due to altered excretion of water-soluble vitamins. 
  • Do not substitute for anti-epileptic or other prescribed medications. 
Contraindications 
  • Hypersensitivity to any component of the formulation 
  • Leber’s disease (for Methylcobalamin) — may worsen optic nerve damage. 
Drug Interaction
  • Anticonvulsants (e.g., phenytoin, phenobarbital) may decrease folate levels. 
  • Chloramphenicol may interfere with the hematologic response to vitamin B12. 
  • Methotrexate, sulfasalazine, and trimethoprim may reduce folate action. 
  • Alcohol may impair absorption and metabolism of B-vitamins.
Pregnancy
  • Generally considered safe during pregnancy. 
  • Folate is essential for neural tube development in fetuses. 
  • Use under medical supervision.
Adverse Effects
  • Gastrointestinal upset (rare) 
  • Allergic reactions like rash or itching (very rare) 
  • Headache, dizziness (in some sensitive individuals)

Overdosage

Pharmacology
L-Carnitine: 
A quaternary ammonium compound that facilitates the transport of long-chain fatty acids into mitochondria for β-oxidation, producing energy. It also exhibits antioxidant properties and supports mitochondrial function, particularly in muscle and nerve cells. 

Methylcobalamin (Vitamin B12): 
The active form of Vitamin B12, essential for neuronal health, DNA synthesis, and erythropoiesis. It promotes myelin sheath formation and nerve regeneration. 

Folic Acid (Vitamin B9): 
Crucial for DNA synthesis, repair, and methylation. Works synergistically with Vitamin B12 in red blood cell formation and prevention of neural tube defects. 

The combination enhances neurological repair, energy production, and haematopoiesis, making it effective in treating neuropathic conditions and deficiencies.

Indications

Hybrin LC Tablet is indicated for: 

  • Peripheral neuropathy 
  • Diabetic neuropathy 
  • Nutritional deficiencies (B12 or folate) 
  • Fatigue and muscle weakness 
  • Male infertility related to mitochondrial dysfunction 
  • Adjunct in cardiovascular health and chronic fatigue syndrome
Dosage
One tablet daily, or as directed by the healthcare provider. Preferably taken after meals to enhance absorption and minimize gastric discomfort.
Precautions
General 
  • Monitor Vitamin B12 and folate levels periodically in long-term use. 
  • Ensure correct diagnosis of neuropathy before starting supplementation.
Warnings
  • High doses of L-Carnitine may cause a fishy body odour in some individuals. 
  • In rare cases, Methylcobalamin may unmask subclinical polycythemia or folate-deficiency-related symptoms. 
  • Do not exceed recommended dose without medical supervision.

Contraindications 
  • Hypersensitivity to any of the active ingredients. 
  • History of Leber's disease (hereditary optic nerve atrophy) – Methylcobalamin may aggravate the condition. 
  • Folic acid should be used cautiously in individuals with untreated Vitamin B12 deficiency due to the risk of neurologic complications.
Drug Interaction
  • L-Carnitine: May interact with warfarin (potentiation of anticoagulant effect). 
  • Methylcobalamin: Absorption reduced by chronic use of proton pump inhibitors, H2 blockers, and metformin. 
  • Folic Acid: May decrease serum phenytoin concentrations.
Pregnancy
  • Category A (Folic Acid, Vitamin B12): Safe when used at recommended doses. 
  • L-Carnitine is generally considered safe, but high doses should be used only under medical supervision during pregnancy.
Adverse Effects
  • Nausea, vomiting, diarrhoea 
  • Headache 
  • Mild gastrointestinal discomfort 
  • Rare: Allergic reactions like rash or itching
Overdosage
  • Overdose is rare and usually non-toxic.

Pharmacology
Methylcobalamin (Vitamin B12): 
Essential for DNA synthesis, myelin sheath integrity, and nerve cell repair. It supports the regeneration of damaged neurons. 

Folic Acid (Vitamin B9): 
Aids in DNA/RNA synthesis and repair, red blood cell formation, and works synergistically with B12 in neurological and haematological health.
