Pharmacology
Trypsin & Chymotrypsin (Proteolytic Enzymes)
Mechanism of Action:
- These are serine proteases that break down peptide bonds in proteins. ∙They help in reducing inflammation, edema, and bruising by digesting abnormal proteins at the inflammation site.
- Facilitate removal of necrotic tissue, improve blood circulation, and promote healing.
- Exhibit fibrinolytic activity, reducing capillary permeability and fluid retention in inflamed tissues.
Absorption:
Administered orally in enteric-coated tablets to prevent degradation in the stomach. Absorbed in the small intestine.
Indications
Tymolytic is indicated for:
- Post-operative inflammation and swelling
- Traumatic injuries (sprains, strains, soft tissue injuries)
- Fracture and orthopaedic recovery
- Sports injuries
- ENT inflammations (sinusitis, tonsillitis)
- Dental trauma/post-extraction swelling
- Wound healing support
- Post-Cesarean and general surgical inflammation
Dosage
- Adults: 1 tablet 4 times daily, preferably on an empty stomach (1 hour before or 2 hours after meals), or as prescribed.
- Children: Use under medical supervision.
Precautions
General
- Administer on an empty stomach to ensure maximum enzyme activity.
- Monitor patients with bleeding tendencies or coagulation disorders.
- Not for use in acute infections without medical advice.
Warnings
- Use cautiously in patients with peptic ulcers, bleeding disorders, or allergic predispositions.
- Discontinue if signs of allergic reaction or GI distress occur.
- Avoid concurrent use with other proteolytic enzymes unless prescribed.
Contraindications
- Hypersensitivity to trypsin or chymotrypsin
- Bleeding disorders
- Peptic ulcer disease
- Severe hepatic or renal dysfunction
- Recent haemorrhagic stroke
Drug Interaction
- Anticoagulants (e.g., warfarin, aspirin) – risk of bleeding may be increased.
- Antibiotics (e.g., amoxicillin) – absorption may be enhanced by enzymatic activity.
- NSAIDs – concurrent use may increase the risk of GI discomfort.
Pregnancy
- Use with caution. Safety not well established.
- Avoid use in late pregnancy due to potential impact on clotting factors.
- Use only if benefits outweigh risks, and always under physician advice.
Adverse Effects
- Gastrointestinal: Nausea, abdominal discomfort, diarrhoea.
- Hypersensitivity: Rash, itching, rarely anaphylaxis.
- Others: Metallic taste, flatulence, headache (rare).
Overdosage
Symptoms: GI upset, nausea, allergic reaction.
Management: Supportive and symptomatic care. No specific antidote.

Pharmacology
Trypsin
- A proteolytic enzyme that helps break down proteins into smaller peptides.
- Reduces inflammation and edema by clearing damaged tissue and improving circulation.
- Aids in tissue repair and wound healing.
Bromelain
- A proteolytic enzyme extracted from pineapple.
- Possesses anti-inflammatory, anti-edematous, and analgesic properties.
- Inhibits bradykinin and prostaglandins, reducing swelling and pain.
- Enhances antibiotic absorption in tissues.
Rutoside (Rutin)
- A flavonoid with potent antioxidant and vasoprotective properties.
- Strengthens capillaries and blood vessels, reducing leakage and bruising.
- Scavenges free radicals and reduces oxidative stress.
Synergy:
- Trypsin and Bromelain reduce inflammation, while Rutoside protects blood vessels and enhances healing. Used widely in post-surgical, orthopedic, and dental recovery.
Indications
Tymolytic BR is indicated in the management of:
- Postoperative inflammation and edema
- Musculoskeletal injuries (sprains, strains, contusions)
- Sports injuries
- Dental trauma and post-extraction swelling
- Sinusitis and ENT inflammatory conditions
- Varicose veins, haemorrhoids, and capillary fragility
- Joint pain, arthritis, and soft tissue inflammation
Dosage
- Adults: 1 tablet twice or thrice daily, preferably 1 hour before meals or as advised by a physician.
- Children: Not recommended unless prescribed by a healthcare professional.
Precautions
General
- Should be taken on an empty stomach for better absorption.
- Not for immediate relief of acute pain.
- Use cautiously in patients with coagulopathies.
Warnings
- Discontinue if any signs of allergic reaction occur.
- May increase risk of bleeding when combined with anticoagulants.
- Bromelain may cause GI upset in sensitive individuals.

Pharmacology
Metronidazole: A nitroimidazole antimicrobial with bactericidal activity against anaerobic bacteria and certain protozoa. It disrupts DNA synthesis in microbial cells.
Povidone Iodine: A broad-spectrum antiseptic that slowly releases iodine, which penetrates microbial cell walls causing oxidation and cell death.
Sucralfate: A cytoprotective agent that forms a protective barrier over ulcers or damaged skin/tissue, promoting healing and regeneration.
The combination provides antimicrobial, anti-inflammatory, and wound healing properties, ideal for treating infected or non-healing wounds and ulcers.
Indications
Healsure Ointment is indicated for:
- Episiotomy
- Infected wounds
- Pressure sores
- Diabetic foot ulcers
- Bed sores
- Non-healing surgical wounds
- Skin ulcers with secondary infection
Dosage
Apply a thin layer of Healsure Ointment to the affected area 1 to 2 times daily, or as directed by the physician. Clean and dry the area before application. Cover with a sterile dressing if required.
Precautions
General
- Avoid contact with eyes and mucous membranes.
- For external use only.
- Use cautiously on large wounds or deep puncture wounds.
Warnings
- Prolonged use may cause iodine absorption or bacterial resistance.
- Not recommended for long-term continuous application on large skin areas.
Contraindications
- Known hypersensitivity to Metronidazole, Iodine, Sucralfate, or any component of the formulation.
- Hyperthyroidism (due to iodine content).
- Use on large burns may not be advisable.
Drug Interaction
- Metronidazole may interact with alcohol (disulfiram-like reaction).
- Povidone Iodine may affect thyroid tests and interact with lithium.
- Sucralfate can bind and reduce the absorption of some topical or systemic medications if used inappropriately.
Pregnancy
- Category B (Metronidazole, Sucralfate), Category D (Povidone Iodine): Use only if clearly needed and prescribed.
- Avoid during the first trimester and near term unless prescribed due to potential iodine absorption and foetal thyroid impact.
Adverse Effects
- Skin irritation, itching, or burning at application site.
- Allergic dermatitis or hypersensitivity reactions.
- Discoloration of the skin due to iodine.
Overdosage
Topical overdose is rare but may result in:
- Local irritation or chemical burns.
- Systemic iodine toxicity (in case of excessive or large area application).
- Seek medical attention in case of ingestion or systemic symptoms.
